Aging and Chronic Diseases by Gregory Guyot:

ELASTIC BAND EXERCISES FOR ELDERLY PEOPLE

(...can they help in reducing or even preventing frailty?)

Frailty is a very common geriatric syndrome which is defined as a clinical state with increased vulnerability when facing unpredicted or undesirable stimulations. It is the consequence of a progressive and multi-factorial age-related physiologic decline affecting several body systems and leading to higher economic burdens, risks of dependency, cognitive issues, falls, hospitalisation or death.

Exercises and physical activity are key interventions to prevent and manage frailty according to literature. Several modes of exercise are recommended, such as balance, aerobic or flexibility. Resistance training seems to be an important aspect to consider and the authors focused here specifically on the effects of elastic band resistance training.

Such intervention indeed shows several advantages for pre-frail elderly people fighting against frailty: enough time to recover, less risk of injuries (safe tool), involvement of multiple muscle groups, improvement of strength but also gait function, balance, flexibility, quality of life, risk of falls…

The authors found that elastic band training improves frailty states in pre-frail elderly people, allowing them to break away from pre-frailty and restoring them to non-frailty through improvement of frailty symptoms.

CHANGING THE NARRATIVE OF SACROILLIACO PAIN.

(How sure can we be when identifying the disorders of the sacroiliac movement? ))

Sacroiliac joint movements (SIJ) are imperceptibly small and there is no evidence to support the use of clinical trials of the dysfunction of the SIJ movement. However, the presence of pathoanatomic processes in SIJ is not denied; they may very well contribute in part to the experience of pain.

A paradigm shift is needed in clinical reasoning, towards modifying doctor s' perceptions, correction of beliefs about structural fragility and the implementation of evidence-based treatment fundamentals. These are conclusions from an international group of scientists who conducted a review on this topic.

WHAT DOES YOUTUBE OFFER ON PAIN EDUCATION VIDEOS?
(…and are those videos reliable? )

What's new
- Seven principles of neuroscientific pain education
- Principles are rarely discussed
- Increased popularity, but reliability hard to determine

YouTube offers of wide variety of pain education videos, but only few discuss at least one of the seven principles of neuroscientific pain education in a fascinating way. American and Australian researchers who analysed 106 YouTube videos conclude that more research is necessary to determine the clinical usability of these videos: what do patients learn from the videos and do they change their behaviour as a result?

Musculoskeletal by José Pedro Correia:

PAIN-FREE VS. THRESHOLD REHABILITATION IN HAMSTRING STRAINS

(Are there advantages to reaching the pain threshold in the rehabilitation of acute hamstring strains?)

Allowing the rehabilitation of acute hamstring strain injuries (HSIs) to be guided by the pain threshold did not reduce the time to return to play (RTP).

However, it did lead to a greater recovery of isometric knee flexion strength and improved biceps femoris long head fascicle length at the 2-month follow-up.

Thus, applying early eccentric loading and long-length exercises to the pain threshold may help manage deficits in knee flexor strength and help maintain gains in biceps femoris fascicle length without influencing the RTP timing.

PELVIC FLOOR MUSCLE TRAINING IN PROLAPSE SURGERY

(...does the addition of perioperative pelvic floor muscle training improve outcomes?)

Pelvic organ prolapse (POP) is a major female health problem with a negative impact on quality of life. The most valid symptom of POP is the sensation of a bulge in the va**na.
Therapeutic options for POP include surgery and conservative treatments such as insertion of a pessary or pelvic floor muscle training (PFMT).

Recent systematic reviews have concluded that PFMT reduces POP symptoms and severity stage and PFMT has been shown to:

Increase pelvic floor muscle (PFM) strength and endurance;
Reduce the levator hiatus area;
Lift the bladder and re**al ampulla;
Increase PFM volume; and reduce PFM length.
However, the recurrence rate after POP surgery is high, ranging from 10 to 54%. Because of this, it could be assumed that the success rate of POP surgery would increase by combining surgery with PFMT.
