Buckle up - The trusty Doppler. Or.. is it?

As if blood pressure monitoring under anaesthesia isn’t at times a random number generator - I always rated Doppler above oscillometric NIBP devices however the evidence suggests it isn’t too trustworthy after all! I was shook.

What is correct? What can we trust? What is the right thing to do? Before I open this can of worms, remember – we should be monitoring the TRENDS and not the number. It is also worth noting a number of these studies I discuss are not comparing apples with apples in terms of using the same monitoring machine. There is a lot of talk around the validation of certain devices on different species in different settings e.g. anaesthetised, conscious, critically ill etc – so I would encourage you to research your devices with the keyword “ACVIM” in PubMed/Google to see how it stacks up.

Right, pass the can opener – lets set 'em free:

In anaesthetised dogs using a Doppler, Seliskar et al., (2013) found that only 10% and 34% of measurements were within 10 and 20mmHg of IBP values, respectively, often with the Doppler overestimating readings. They also found that the HDO device has better agreements with IBP mean values as 95% of readings were within 20mmHg. So should we favour our oscillometric machine more under anaesthesia? Well, not too quickly.. Shih et al., (2010) found that an oscillometric device did not reliably predict hypotension associated with acute haemorrhage. Additionally, when comparing the Doppler to IBP readings in anaesthetised dogs, Bourazak and Hofmeister (2018) found that the Doppler couldn’t reliably detect hypotension (Systolic

CONSTANT RATE INFUSION (CRI) FOR CLUSTER SEIZURES (CS) OR STATUS EPILEPTICUS (SE)
Nice retrospective study freshly published in Frontiers in Veterinary Science on the use of CRI of diazepam (DZP) or propofol (PPF) for canine CS or SE. The 37 dogs included in this case series included dogs with idiopathic epilepsy, structural epilepsy as well as metabolic or toxic cause of seizure activity. The authors carefully noted some limitations of this study such as absence of randomisation in the choice of CRI between DZP and PPF as well as clinical efficacy being defined as cessation of physical manifestation of seizure activity rather than EEG burst suppression. This study however document the use of CRI of DZP or PPF in a large sample of dogs with various causes of seizures.

By skutecznie zapobiegać wypaleniu zawodowemu i podnosić swoje kompetencje warto doskonalić swoją wiedzę i umiejętności w obszarach: komunikacji interpersonalnej, budowania asertywnej postawy i komunikacji, zarządzaniu czasem, zarządzaniu stresem oraz współpracy w zespole.
Wypaleniu zawodowemu można i trzeba zapobiegać, a jeżeli już profesjonalista znajdzie się na tej ścieżce, to można z niej zawrócić. Kluczowe w tym procesie jest budowanie ww. kompetencji osobistych.
Dlatego warto wiedzieć czym kompetencje osobiste dla lekarza weterynarii są, jaką mają rolę w zapobieganiu wypaleniu zawodowemu, czy i jak można je doskonalić i co dobrego podnoszenie kwalifikacji w tym obszarze nam przynosi. Więcej w tych tematach na konferencji. Zapraszamy!

HOW TO FEED YOUR CAT AND AVOID KIDNEY FAILURE

Kidney disease is the leading cause of death in older cats. 😿Typically a slowly progressive disease, many cat owners don’t notice any signs of kidney failure until it is too late. It’s important to protect your cat’s kidney function throughout life, and one science-backed way is to incorporate a good quality DHA/EPA (ocean-sourced omega 3🐟) into your cat’s daily routine.

Essential fatty acids are particularly useful for cats with kidney disease. Researchers👩‍🔬 at Colorado State University found that DHA may decrease inflammation in chronic kidney failure and that higher DHA intakes may protect against malnutrition. In addition to reducing kidney inflammation, fish body oils help retain proteins and lower high blood pressure, helping cats with kidney disease live longer lives.🙌

APPROACH TO CANINE EPILEPTIC SEIZURES IN PRIMARY CARE PRACTICES
We are grateful to BSAVA PetSavers for funding this study we recently published in JSAP. Matthew Green, neurology resident at Dovecote Veterinary Hospital and lead investigator in this study, looked at epileptic dogs in first opinion practice to see what our colleagues do for management. 517 dogs were included in this study from Savsnet - University of Liverpool.
• 321 dogs presented for 1 seizure; 7 started on AEDs
• 86 presented for clusters; 50% started on AEDs
• Imepitoin most frequently used for clusters despite no license for this
• 165 dogs presented for 1 seizure+>6month follow-up - 20% of these had no more seizures
