SPORTS PHYSICALS NIGHT COMING UP FOR ABILENE STUDENTS

The annual sports physical clinic will be held on Wednesday, April 8, 2026 at Memorial Hospital's Heartland Health Care Clinic, in Abilene, from 5:00 p.m. to 7:00 p.m.

Who: Current U.S.D. #435 students who will be in grades 7-12 in the 2026-27 school year who would like to participate in school sports next year. (Cross Country, Football, Cheer, Dance, Tennis, Volleyball, Basketball, Wrestling, Powerlifting, Baseball, Softball, Golf, Track).

What: Annual Sports Physical Clinic
Where: Memorial Hospital—Heartland Health Care Clinic
When: Wednesday, April 8, 2026 from 5:00 p.m. to 7:00 p.m.

Cost: $10 Cash only. No checks, no debit/credit cards, no insurance will be filed.

*Paperwork may downloaded from the school nurse page on the school website, and will be available in the school offices (KSHSAA has updated the form for 2026-27).

*Only school athletic physical forms will be completed at this clinic. Camp forms and other outside forms will not be able to be completed.

*Packets must be completed and all parent and student signatures must be in place prior to getting in line.

We strongly encourage all students who may be considering participating in school athletics next year to take advantage of this opportunity. All students who participate MUST have a valid sports physical and required forms dated after April 1, 2026 (using the new KSHSAA form dated 9/2025 at the bottom—old forms cannot be accepted). These must be on file with the school prior to being allowed to practice for the 2026-27 school year.

Physician offices are very busy, and it can be very difficult to get an appointment prior to practices beginning. This means your student will have to sit out until an appointment can be completed.
