At 96 years old, this was no ordinary case.

A former physician herself, she came to Dr. Ayzenberg with a nearly non-functional shoulder and severe, debilitating pain that had taken over her daily life. After failing extensive conservative care and relying on narcotics without relief, surgery became the only path forward—but at her age, every detail matters.

This procedure required an exceptional level of precision, planning, and care. Bone quality, tissue integrity, and overall health all make shoulder surgery at 96 incredibly delicate—where even small decisions can have a major impact on outcome.

It’s a powerful example of what modern orthopaedic surgery can achieve when technique and experience come together.

Two months ago, Ken underwent surgery for a complex distal humerus fracture.

Today he’s already demonstrating excellent range of motion in his elbow—straightening, bending, and rotating with impressive control and function.

Early recovery like this doesn’t happen by accident. It’s the result of precise surgical technique, the right fixation, and a dedicated rehabilitation process.

At Iron Mountain Orthopaedic Institute, Dr. Ayzenberg focuses on restoring not just healing—but real, functional movement that gets patients back to living their lives.

Recovery is a process. And in cases like Ken’s, progress can come faster than most expect.

Casually sharing ice cream with one of the true pioneers in shoulder surgery.

Here I am with Dr. Stephen J. Snyder—a leader in orthopaedic sports medicine and widely recognized as one of the fathers of shoulder arthroscopy. From developing groundbreaking surgical techniques to inventing instruments used in surgeries around the world, Dr. Snyder’s impact on modern shoulder care is unmatched.

During my Sports, Arthroscopy and Reconstructive Surgery fellowship at the Southern California Orthopedic Institute, I had the privilege of learning directly from Dr. Snyder.

Grateful to have trained with the best—and to bring those same techniques and standards of care to patients here in Pennsylvania.

What happens when a surgeon uses the same precision with photography as he does in the operating room? Photos like these.

Rising from the tidal flats of Normandy, Mont Saint-Michel is one of France’s most iconic landmarks — a centuries-old abbey perched atop a rocky island, surrounded by water at high tide and vast sands at low tide. Its changing surroundings make every moment feel entirely unique.

Dr. Ayzenberg took two long-exposure shots of this beautiful landmark - one right as the sun was setting, and the other was just after the sun set and the colors started changing.

Which one is your favorite?

Dr. Evan O’Brien — an orthopaedic spine surgeon and active CrossFit athlete — was dealing with bone-on-bone arthritis in his knee.

After a scope procedure elsewhere, things didn’t improve… they got worse.
Pain. Limping. Losing the ability to do what he loves.

Instead of jumping straight to a total knee replacement, he came to Dr. Ayzenberg to explore stem cell therapy.

At Iron Mountain Orthopaedic Institute, Dr. Ayzenberg performed a bone marrow–derived stem cell procedure, precisely targeting the area beneath the cartilage damage.

The result?

✔️ No longer limping
✔️ Returned to CrossFit and daily activity
✔️ Significantly improved function in a matter of months
✔️ Avoided knee replacement surgery

At Iron Mountain Orthopaedic Institute, we focus on advanced, targeted treatments that may help you avoid surgery — and get back to doing what you love.

If you're struggling with joint pain or injuries, schedule a call with Dr. Ayzenberg's team to see how we can help you.

Rebuilding your knee after surgery starts with the fundamentals.

In this video, Dr. Ayzenberg walks through key exercises to help you regain strength and motion safely during recovery.

✔️ Quad activation to maintain muscle strength and prevent atrophy
✔️ Controlled knee straightening — even small improvements matter
✔️ Gradual bending exercises to restore range of motion over time
✔️ Simple techniques like heel slides and assisted hangs to support progress

Every recovery is different, and not every exercise applies to every surgery — which is why following your specific post-op plan is critical.

The goal isn’t to rush. It’s to progress steadily, safely, and effectively so you can get back to full function.

Elva came in with a massive, three-tendon rotator cuff tear — significantly retracted and atrophic.

This is the kind of injury many people are told will require a shoulder replacement.

But today?

✔️ Excellent range of motion in all directions
✔️ Strong, functional movement behind the back and overhead
✔️ Minimal discomfort
✔️ Back to real-life activity — building a deck, doing hardwood floors, yard work, and more

MRI even confirms healing — despite the severity of the original tear.

“I can do just about anything.”

This is what the right approach — and the right patient — can achieve in a matter of months.

At Iron Mountain Orthopaedic Institute, Dr. Ayzenberg focuses on restoring function and helping patients get back to the life they want — without jumping straight to joint replacement.

At 96 years old, she was in constant pain—relying on narcotics just to get through the day. Even basic movement was a struggle.

Just weeks after surgery?
She’s pain-free, off narcotics, and already regaining function in her shoulder.

This is why we do what we do.

Her procedure was a complex reverse total shoulder replacement performed by Dr. Ayzenberg—captured using the Apple Vision Pro to provide an immersive, detailed view of the surgery. The case also utilized a custom glenoid component from the Zimmer Biomet VRS Arthroplasty System, allowing for a highly personalized approach.

Technology is advancing—but what matters most is the outcome: real patients getting their lives back.

There’s a different kind of precision behind the lens.

For Dr. Ayzenberg, landscape photography is where creativity meets intention — a space to slow down and capture the world with the same attention to detail he brings to surgery. This image, taken during a 2022 trip the lavender fields of Provence, France, reflects his passion for vibrant color, depth, and the natural beauty of a moment.

His work is produced on metal and other media, giving each piece a striking sense of dimension, and is displayed in commercial buildings, local venues, and homes internationally. To create these images, Dr. Ayzenberg often merges multiple photos and carefully enhances them in post-processing — not to alter the landscape, but to bring out its most breathtaking qualities.

Zach was told surgery was the only option for his shoulder.

He chose to explore another path.

Instead of going straight to surgery, he started with PRP therapy — not just for his shoulder, but also for his knee, which had “bone-on-bone” arthritis.

The results?
✔️ Shoulder pain resolved with full range of motion restored
✔️ Knee improved significantly with PRP
✔️ Over a year later, his PRP results were still maintained — but wanting to maximize his outcome, he opted for stem cell treatment, leading to another dramatic improvement in his knee

In fact, while PRP worked very well, the addition of stem cells for his knee took his recovery to the next level.

And not everything required injections — his ankle was successfully treated with physical therapy alone, reinforcing that the right treatment depends on the specific condition.

At Iron Mountain Orthopaedic Institute, Dr. Ayzenberg focuses on using the right tools for the right problem — whether that’s PRP, stem cells, physical therapy, or a combination — with the goal of helping you avoid surgery whenever possible.

Are Bone Health Medications Increasing Your Fracture Risk?

Most people think bone-strengthening medications are a long-term solution… but there’s an important detail many don’t hear about.

Medications like Fosamax (and other bisphosphonates) can be very effective early on. But after 3–5 years, staying on them continuously may actually increase the risk of rare, unusual hip fractures.

That’s why doctors often recommend something called a “drug holiday.”
➡️ Pause the medication for a period of time
➡️ Then restart later if needed
➡️ Help reduce long-term fracture risk

The key takeaway?
Bone health isn’t just about taking medication — it’s about managing it the right way over time.

If you’ve been on a bisphosphonate for years without a check-in, it may be time to revisit your plan.

At Iron Mountain Orthopaedic Institute, we focus on personalized strategies to help protect your bones — now and long-term.
