If you've ever had an injury, you know the feelings of fear, isolation, frustration, anxiety, and so much more that come and go throughout the recovery process.

Everyone talks about the physical aspects of recovery from injury and surgery, but we can't forget about the psychological factors. When left unaddressed, these factors can limit progress in strength and function, delay return to sport/activity, and even increase the risk of re-injury.

These are a few of our favorite tips for our patients:

🎉Celebrate the big (and little) wins! Take pictures and videos throughout the process so you can look back and see all you've accomplished.

✍️Write it out. Try starting a journal to keep track of your progress and organize your thoughts and feelings.

👯‍♀️Choose someone to be your "go to". Social support is really important throughout this process and often you are removed from your typical support system

Whether you’ve had a knee injury or you know someone who’s undergone knee surgery, we all know what happens...the quadriceps muscle shrinks away in what feels like minutes. And often stays that way for a long time.

Ever wondered why it takes so long for the quadriceps muscle to regain strength and size after a knee injury like an ACL tear?

There’s a difference between muscle weakness and muscle inhibition...

💪🏽 If someone with a healthy neuromuscular system (no injury) started an exercise program, they would see good results in as little as 6 to 8 weeks. A weak muscle gets strong quickly.

🦵🏼As someone with an ACL injury completes rigorous rehabilitation, the quadriceps muscle does not respond in terms of strength or volume. An inhibited muscle cannot fully activate, so exercise alone is not enough.

🔬 New Research by our Team on the topic of Quadriceps Weakness Post-ACL Reconstruction

📚 "Neural Drive and Motor Unit Characteristics after Anterior Cruciate Ligament Reconstruction: Implications for Quadriceps Weakness"

🎯 Purpose: The study aims to understand the neural and muscular factors contributing to persistent quadriceps weakness in individuals who have undergone Anterior Cruciate Ligament Reconstruction (ACLR).

🔍 Methods: 14 individuals with ACLR and 13 matched controls participated. Various contraction intensities were tested using decomposition electromyography (dEMG) and torque measurements. Hoffmann reflex (H-reflex) and Central Activation Ratio (CAR) were also assessed.

📊 Key Findings:
Lower Torque in ACLR: Individuals with ACLR showed lower maximal voluntary isometric contraction (MVIC) torque in the involved limb compared to the uninvolved limb.
No Difference in H-reflex or CAR: No significant differences were found in H-reflex or CAR between the groups or limbs, indicating activation of a similar proportion of the motor neuron pool.
Slower Firing Rates: The involved limb in the ACLR group had slower motor unit firing rates at high contraction intensities (70% and 100% MVIC) compared to uninvolved and control limbs.
Larger Action Potentials: The involved limb in the ACLR group also showed larger motor unit action potentials compared to the contralateral limb.

📝 Conclusions:
The results suggests that the persistent strength deficits following ACLR may be due to a diminished quadriceps motor neuron pool and an inability to upregulate the firing rate of recruited motor units.

Healthcare professionals and rehabilitation experts should consider these neural and muscular factors when designing post-ACLR rehabilitation programs.
