CHP Home Care & Hospice is a community-based, nonprofit organization serving northwest and west-central Ohio since 1975. Our services are Medicare and Medicaid certified and accredited by CHAP. What makes CHP unique is that we provide a complete range of services - including Visiting Nurses, In-Home Therapy, Hospice, Inpatient Hospice, Private Duty, and Adult Day Care. Our nurses and staff are local and available 24-hours a day, seven days a week. Our staff will help you decide what service is right for you and we will work with your physician. We are happy to provide a no-obligation, informational visit by one of our friendly and caring nurses. She will be happy to answer any questions you may have.
