CHP Home Care & Hospice is a community-based, nonprofit organization serving northwest and west-central Ohio since 1975. What makes CHP unique is that we provide a complete range of services - including Visiting Nurses, In-Home Therapy, Hospice, Inpatient Hospice, Private Duty, and Adult Day Care. Our nurses and staff are local and available 24-hours a day, seven days a week. Making a referral to CHP is easy...just call us! Our staff will help you decide what service is right for you and we will work with your physician. We are happy to provide a no-obligation, informational visit by one of our friendly and caring nurses. She will be happy to answer any questions you may have.
