It is that time of year already! Schedule your sports physical so you can ✅that off your list and enjoy the rest of summer! Typically needed for athletes going into 7th and 10th grade. Make sure to bring your $30 payment and the 25-26 Sports Physical form with you. Please complete top & bottom of page 1 and all of page 2 before your appointment.
