📢 Important Update for Our Patients 📢

We recently transitioned to a new EMR system that sends all required paperwork electronically to the email you provide when scheduling.

When you schedule an Initial Evaluation, you will receive an email with the current forms you need. We are in process of removing the paper forms off our website, please do NOT print those and bring those to your appointment. They are no longer valid.

We kindly ask for your patience and understanding with our front desk team and staff during this transition. This is a significant change for our office, and while we are excited about the improvements, we are still working out a few kinks. Your kindness and cooperation mean the world to us! 💙
