Hospice del Valle is now hiring a full-time Nurse Case Manager.

Requirements include:
* Graduate of an accredited school of professional nursing
* Current licensing as a Registered Nurse in Colorado
* BSN preferred
* At least one year of professional experience in the delivery of nursing services, two to five years preferred
* Willingness and ability to travel within the hospice service area
* Ability to lift fifty pounds and independently perform personal care on a patient who weighs up to two hundred and fifty pounds.

The Nurse Case Manager provides leadership abilities with strengths in the following areas:
* Understanding, identifying, and assisting in the coordination of the needs of the terminally ill and their families with a strong commitment to the hospice concept
* Ability to understand and promote effective interaction between the members of the IDT, other hospice members, nursing homes, hospitals, and community members
* Effective verbal, nonverbal, and written communication skills
* Supervision of CNAs, LPNs, and other paraprofessionals providing care to the patient and family
* Physical and emotional health and personal qualities which enable him/her to work effectively with patients, families, and staff, including satisfactory adjustment to significant personal losses
* A strong desire for continued personal and professional growth and development
* The ability to perform duties independently
* The ability to work non-judgmentally with patients and families of many cultural, religious, and socioeconomic backgrounds and a variety of lifestyles.
