⚡ The difference between “watching” and “coding” a patient.

ECG interpretation can be intimidating, but recognizing lethal rhythms within seconds saves lives.

✅ Normal Sinus Rhythm (NSR):
P wave before every QRS, regular rhythm, rate 60–100 bpm — this is what we want.

⚠️ V-Tach (Ventricular Tachycardia):
Wide, fast complexes — often described as “tombstones.”

Rule: ALWAYS check for a pulse.
• Pulse present + unstable patient → Synchronized cardioversion
• Pulse present + stable → Antiarrhythmics & expert consult
• No pulse → Start CPR and DEFIBRILLATE

⚡ V-Fib (Ventricular Fibrillation):
Chaotic, quivering rhythm — no cardiac output.

Rule: This is a cardiac arrest.
Action: Immediate DEFIBRILLATION + CPR

🚫 Asystole (Flatline):
No electrical activity.

Rule: Asystole is NOT a shockable rhythm.
Action: High-quality CPR, Epinephrine, and treat reversible causes (Hs & Ts)

🛑 Are you taking your PPE off WRONG? (This is how we get sick!)

Putting on (Donning) and taking off (Doffing) your PPE is one of the most tested subjects on the NCLEX and the most critical skill on an isolation floor.

If you take your mask off before your dirty gloves... you just contaminated your face! 🦠

Here is the exact CDC Official Sequence you need to memorize:

⬆️ DONNING (Putting It ON): Work from the bottom up!

• Gown: Tie it securely in the back.

• Mask/N95: Secure the ties/bands at the middle of head and neck.

• Goggles/Face Shield: Place over face and adjust.

• Gloves: CRITICAL STEP - Ensure the gloves extend OVER the cuffs of the isolation gown so no skin is exposed!

⬇️ DOFFING (Taking It OFF): The Rule: The DIRTIEST things come off first!

• Gloves: These touched the patient. Take them off first!

• Goggles/Face Shield: Remove from the back by lifting the headband (the front is contaminated).

• Gown: Unfasten ties, pull away from neck and shoulders, touching the inside only. Roll it into a bundle.

• Mask/N95: CRITICAL STEP - Do not remove this until you have LEFT the patient's room and closed the door!

🫀 Are you pushing deep enough? The CPR rules you cannot forget! 🚨

When a patient codes, muscle memory has to take over. You don't have time to look up the AHA (American Heart Association) guidelines. But treating a 6-month-old baby the same way you treat a 60-year-old adult will lead to catastrophic injuries.

Here is the exact breakdown of Adult vs. Infant CPR you need for your BLS certification and the floor:

🧑 ADULT CPR (Puberty and older):

• The Depth: You must press down at least 2 inches (but no more than 2.4 inches). If you aren't breaking a sweat (and sometimes hearing ribs crack), you probably aren't going deep enough to squeeze the heart!

• The Ratio: 30 compressions to 2 breaths (30:2) for both 1-rescuer and 2-rescuer CPR.

👶 INFANT CPR (Under 1 year old):

• The Depth: Only press about 1.5 inches (or 1/3 the depth of the chest).

• The Technique: Use 2 fingers in the center of the chest (just below the ni**le line) if you are alone. If there are two of you, use the 2-thumb encircling hands technique!

• The Ratio Trap: If you are alone, it is 30:2. But if you have another nurse with you, it switches to 15:2!

⚡ The Golden Rule for BOTH: Always compress at a rate of 100 to 120 beats per minute. (Sing "Stayin' Alive" by the Bee Gees in your head!)
