Care Design NY is a Care Coordination Organization/Health Home (CCO/HH) in New York State. We provide person-centered comprehensive care management services for 27,000 individuals with intellectual and/or developmental disabilities (I/DD) in 30 New York State counties, so they may lead a quality life. We utilize a person-centered and holistic service planning approach to coordinate care and deliver integrated supports and services with individual, family and provider input.
More than 70 agencies in 30 counties with long and successful histories of specializing in the provision of services to children and adults with I/DD came together to create Care Design NY. Our mission is to provide high quality, conflict-free Medicaid Service Coordination and Care Management into one integrated and individualized Life Plan with services and supports authorized by OPWDD.
We have aligned with Partners Health Plan (PHP), which is currently the only provider-led managed care plan solely supporting individuals with intellectual and/or developmental disabilities in the Country. Under this alignment, Partnerships, the parent organization to PHP, will also become the parent to Care Design NY. This makes Partners Health Plan and Care Design NY, “sister agencies.”
This alignment is consistent with New York State’s vision for the move to managed care by developing strong, provider-led managed care plans built from the foundation CCOs have laid for comprehensive care coordination and the investments they are making for managed-care start-up costs. We will be strengthened by supporting each other and sharing resources to be more effective and efficient.

Care Coordination/Health Home (CCO/HH) Services and Supports
Care Design NY will support individuals and families by designing a unique “Life Plan” to incorporate the services and supports that reflects current needs and future life goals. A Care Design NY Care Manager will build upon the foundation created by service coordination to create and deliver a more comprehensive coordination of Health Home (HH) services and supports for individuals so they may lead a quality life. These include:
Assessments to identify medical, behavioral, and social and community services.
Integrated and person-centered Care Management.
Care Coordination and Health Promotion.
Comprehensive Transitional Care.
Individual and family Support.
Referral to Community and Social Support Services.
Development of a tailored Life Plan to meet needs and achieve desired outcomes.

Chief Executive Officer - James F. Moran
Jim has many years of experience supporting and advocating on behalf of individuals with I/DD and their families in New York State, most of which were as part of the Executive team within the Office for People With Developmental Disabilities (OPWDD).
Chief Operating Officer - Anne Ogden, LMSW
Anne has dedicated more than twenty-five years to the community of individuals with I/DD and their families. She has been a leader in providing individualized and person-centered supports most recently as the Executive Director of Catholic Charities Disabilities Services.

Each member of the Board comes from an organization, founded by families, with a rich history of providing supports and services to individuals with IDD and their families.
