It’s been an extra cold winter, making it essential to take steps to stay warm and protect your heart — especially for individuals with I/DD. 🥶 Cold air makes blood vessels in the heart tighten to keep your body warm. When this happens, blood has a harder time flowing.

Care Design NY families and team members recently attended the CMANY Lobby Day alongside an incredible group of advocates, leaders and families — reaffirming our commitment to restoring Care Coordination Organizations (CCOs) to the Human Services Targeted Inflationary Increase.

Without this crucial funding, teams across New York state will feel significant pressures and service instability will continue to grow — impacting the individuals and families who rely on care managers as their primary support.

Joining us were Bin Feng, Jacqueline Fan and Coco Lin — each representing the Chinese-American Planning Council (CPC) and CDNY — alongside team members Megan Long, Tara Giuliano, Don Morse, Danielle Mazza and Cathy Varano.

🚨 It's time to take action.

As a family navigating the New York health system to ensure your child with I/DD can thrive, you understand how crucial a strong care coordination program is.

Unfortunately, New York’s CCO’s, including Care Design NY, are excluded from the current Executive Budget proposal that is due on February 13. We are asking the Legislature to restore CCOs to the Human Services Targeted Inflationary Increase so care management agencies can:
✅ Maintain a stable workforce
✅ Address rising operational costs
✅ Continue providing essential coordination services to approximately 135,000 New Yorkers with I/DD

Without this restoration, workforce pressures and service instability will continue to grow, directly impacting individuals and families who rely on care managers as their primary system.

Our Story

Care Design NY is a Care Coordination Organization/Health Home (CCO/HH) in New York State. We provide person-centered comprehensive care management services for 27,000 individuals with intellectual and/or developmental disabilities (I/DD) in 30 New York State counties, so they may lead a quality life. We utilize a person-centered and holistic service planning approach to coordinate care and deliver integrated supports and services with individual, family and provider input.

Who are we?

More than 70 agencies in 30 counties with long and successful histories of specializing in the provision of services to children and adults with I/DD came together to create Care Design NY. Our mission is to provide high quality, conflict-free Medicaid Service Coordination and Care Management into one integrated and individualized Life Plan with services and supports authorized by OPWDD.
