As you may already know, Albemarle Women’s Clinic, P.A. closed on 08/08/2024. It has been an honor and privilege to have been your health care provider. We are grateful for the opportunity to have served your medical needs.

After closing, your health records from this Practice will be transferred to Morgan Records Management, LLC (the “Records Custodian”). The Records Custodian will continue to maintain your records in accordance with applicable confidentiality and security standards and with other applicable laws. Your records will be available for eleven years.

You may submit a request to the Records Custodian:

There may be a transition period from the time we close to the time Morgan Records Management has access to our charts.

Albemarle Women's Clinic, P.A. will close permanently on 08/08/2024.

Please be advised, I will be closing my Medical Practice on 08/08/2024. I will not be able to provide you with Medical Care after that date.

Medications WILL NOT be prescribed or refilled after 08/08/2024.

I recommend that you find another physician to take care of you. If you do not know another physician, you may contact the NC Medical Society or consult a local physician locator service for a list of physicians.

After the closing, your Medical Records will be available through a Medical Record Custodian Company. The details of requesting your records will be posted at a later date.

September is Ovarian Cancer Awareness Month
