Anyone else feel this way? I was raised super conservative on the intimacy department and I learned a ton from my profession. Since starting my career and practice I have worked with people of many backgrounds to help them find relief with pelvic floor symptoms with physical intimacy.

My goal has been to help people work through pelvic health related symptoms and meet them where they are with the knowledge they have of their bodies and make sense with their beliefs. It can be a sensitive area to talk about, but there is never any judgement and only ways to grow and feel empowered about your body.

Understanding your pelvic floor tone is huge when understanding the best course of treatment for your pelvic floor symptoms. Also keep in mind that knowing the strength, endurance, and coordination is important to know how your muscles respond to commands you tell your muscles. Plus, knowing the “why” for the reason your muscles do what they do.

This helps us to create a specific treatment plan for your pelvic floor and your symptoms. No cookie cutter treatment, but rather an individualized plan just for you.

What happens when we are in a state of stress?
Our bodies prepare to protect itself by, which is good short term.
➡️Vasodilation of eyes to see better.
➡️ increase breathing to get more oxygen for organs that need it such as brain and muscles
➡️ heart rate increases to prepare to move
➡️ muscles are on guard to flee or fight
➡️ hormones change to prepare, such as cortisol and adrenaline (good in short durations but not chronically)
➡️ decreased sleep which can affect our body’s recovery process

As a result we can see:
Constipation, increased urgency or frequency of p*e or p**p, pelvic pain, or other pain (ie headaches).

Stressors we have seen trigger symptoms for our patients:
Losses of loved ones.
Relationship challenges.
Worries about children.
Being stretched too thin.
Multiple small stressors.
Being in a rush all the time.
Plus so many others.

What do we do? Know what your stressors are and have a plan for them.

If you are having physical symptoms get a medical exam to see how all body systems are working. If you have pain or pelvic floor symptoms also work with a pelvic PT to address the physical component of your symptoms.

Common reasons in the adult pelvic floor PT setting to not do internal pelvic floor treatments are:
1. Recently post operative or postpartum. Typically the first 6 weeks is crucial to tissue healing and to prevent risk of infections.

2. Active infections such as UTI, yeast infections, BV, etc

3. Because the patient doesn’t want to. No reason needed. A recent new patient opted to not because of a history of a painful pelvic exam and status as a virgin.

4. You no longer need direct work to the pelvic floor muscles.

What do we do instead?
✅Educate as much as we can about your condition.
✅Teach you how to do things at home for direct pelvic floor work if you don’t want anyone else touching.
✅At least get a visual of what is happening to better guide you on what you can do.
✅Treat all the external areas that can affect your pelvic floor. ✅Teach you exercises and daily strategies to help your pelvic floor.

Pelvic wands can be a great therapeutic tool to help with any pelvic pain, incomplete emptying of p*e or p**p, or to reduce pelvic floor hypertonicity or tightness. People with leakage may have pelvic floor hypertonicity, which they may be a good candidate for focusing on pelvic floor relaxation techniques.
