Meet D. Aaron James, RN
Aaron James, RN moved to Albuquerque in 1998 and found his new home. While Aaron enjoyed making a difference in the acute care setting such as the Emergency Department, the Medical and Cardiac ICU, and enjoyed Flight Nursing for many years, he found his true calling in 2018 when he joined Adam and Melanie in providing hospice care for the New Mexico Community. Hospice Care brought out the compassion, drive and fulfillment that revitalized his purpose and calling for the New Mexico Community. He found that he was passionate about providing exceptional care and going the extra mile to make sure every patient's limited time was as comfortable and meaningful as possible for them and for their loved ones.

Is Hospice the right choice?

Hospice care is health care that is an insurance benefit as well as a philosophy of care.

Hospice focuses on easing the suffering of terminally ill patients and their loved one. This suffering may be physical, emotional, social or spiritual in nature.

Hospice care strives to achieve the best quality day to day life and is a transition from focusing on life prolongation and curative treatments to a focus on comfort and quality of life .

Choosing hospice is not giving up; it is changing the focus of the care received to one that prioritizes the comfort and wellbeing of patients and their loved ones. The hospice team has highly trained doctors, nurse practitioners and nurses who provide medical symptom management and psychosocial support.​

Your home hospice team includes:
- A board certified Hospice and Palliative medicine physician
- A nurse assigned to you with 24/7 access to care by your nurse/physician team and on call providers
- Grief support and counseling from the time of initiation of hospice (anticipatory grief) and for at least 13 months after the death of a loved one.
- A social worker trained to help with any financial or social needs
- A chaplain for spiritual needs
- Hospice aides to help with bathing or other personal care needs
- Volunteers for anything from reading to you or even small errands

When is the right time?

Studies show that most people enroll in hospice too late. Those studies also show that a patient receives the maximum benefit of hospice, on average, after being enrolled for 3 months. The best time to seek hospice services is when you and your loved ones are ready and when your healthcare providers think you qualify. You can self refer for an informational meeting with hospice to learn more and see if you or your loved one qualifies by calling our office.

Meet Melanie Theriot, RN. She moved to Albuquerque at the age of 7 and considers herself a native New Mexican. She has worked with the Geriatric population since 2010 as both a Resident Aide and RN. She has been in hospice for the past 10 years as a Nurse Case Manager, Admissions Nurse, Patient Care Manager, and Director of Clinical Services. She is passionate about hospice because this is a time in people’s lives when they need extra support for themselves and their loved ones. She loves that the approach is holistic and involves an interdisciplinary team. She firmly believes that family members and that the person under care is in charge and decisions are made as a group. Everyone deserves compassion and dignity, and it is her goal to see that each person receives it. Melanie is especially skilled at educating and supporting her team members.

🌿✨ **Wreath-Making for Our Veterans** ✨🌿

Every Year, Our team comes together for a special wreath-creating activity as we work toward our annual end-of-year wreath giveaway to the VA Hospital.

This meaningful tradition allows us to extend our hospice heart beyond our patients and into the veteran community we proudly serve. ❤️🇺🇸 It’s one small way we can show gratitude and honor to those who have given so much.

Our goal this year is to create **500–600 wreaths** by year’s end. With the guidance of our wonderful office volunteers, we are carefully prepping materials and assembling each wreath with care and appreciation.

No special creative skills required — just heart, teamwork, and a shared commitment to serving our community. We’re proud of this tradition and the difference it makes for the veterans we serve. 🌟

Meet Adam Roach, RN, BSN. Adam was born and raised in Melbourne Australia. He moved to the United States 24 years ago to experience travel nursing in hospital emergency rooms, and found his home in Albuquerque NM two years later. He has over 18 years of Hospice experience and feels he has found his life’s passion. That is to support patients, families and caregivers in navigating and understanding the end of life journey. Adam’s goal is to provide our community with the premier Hospice service. He has worked in multiple roles during his tenure and has over 12 years experience as Executive Director. 2. Whole Person Care: Bosque Trails Hospice values who you are and what is important to you in this time. We understand that this journey is different for everyone and we will work towards your personal goals and support you and your loved ones during this time.

3. Clinical Excellence: Our staff at Bosque Trails Hospice is highly trained in the field of Hospice Medicine. All of our physicians are Board Certified in Hospice and Palliative Medicine and our Nurse Practitioner is certified in Palliative Medicine.
