A Patient Centered Medical Home is an approach to providing comprehensive primary care for children, youth, and adults. The PCMH is a healthcare setting that facilitates partnerships between individual patients, their personal healthcare providers, and when appropriate, the patient's family. The provisions of medical home allow better access to health care, increased satisfaction with care and improved health. Family Medicine/ Osteopathic Medicine/ Internal Medicine/ Obstetrics/ Med-Peds/ Behavioral Health/ Dental Health/ and school-based healthcare services. In addition to this, we offer health education on most any health condition you may require.

Living Well with Diabetes class covers carbs, meal planning, medications, blood sugar monitoring, and much more! The class is led by our Registered Dietitian and Registered Nurse. Classes are held online on Thursday afternoons throughout February and March.

From extreme cold and heavy snow to ice storms and heat waves, climate change is making severe weather events more common. While you’re doing everything you can to keep your family safe and warm, remember to keep safety top of mind, too.

🔥 Improper use of generators or fuel‑burning devices can lead to carbon monoxide poisoning.
❄️ Extreme cold can cause frostbite and hypothermia.
💙 Stressful weather events can also affect mental health and well‑being.
