Leucovorin works by bypassing blocked folate pathways in the brain:
• Folate Transport: Normally, folate enters the brain via folate receptor alpha (FRα). In some autistic individuals, autoantibodies block this receptor.
• Leucovorin’s Role: As folinic acid, leucovorin can cross the blood-brain barrier even when FRα is blocked, restoring folate levels in the cerebrospinal fluid.
• Neurological Impact: Folate is essential for:
• Neurotransmitter synthesis (serotonin, dopamine, norepinephrine)
• DNA/RNA production
• Brain development and repair

Q: Is leucovorin FDA-approved for autism?
A: As of September 2025, the FDA is moving to approve leucovorin for treating cerebral folate deficiency and speech-related deficits in autistic children.
