The cost of long-term services and supports (LTSS) increased sharply between 2019 and 2024, erasing roughly a decade of progress in affordability for middle-income older adults. Costs increased most rapidly for widely used services, such as home care and assisted living, rising by nearly 50 percent over five years and far outpacing income growth among older households. A new report by the AARP Public Policy Institute examines state and national trends in LTSS cost and affordability. Findings show a wide variation among states. For every type of service, costs in the most expensive states are at least double those in the least expensive, leaving many families with insufficient income or savings to meet long-term care needs.
LTSS affordability declined as home care and assisted living costs rose nearly 50% from 2019–2024, outpacing income for middle income Americans.
