62nd Annual Scientific Meeting of Midwestern Association of Plastic Surgeons was a blast with record number of attendance and highly scientific content. It was also a wonderful opportunity to catch up with my colleagues and my mentors. See you next year in Lake Geneva.

⚠️Sensitive Content⚠️
This is an 85 year old woman who previously had a right knee replacement. She fell at home and had large bleeding in her knee (hematoma). Necessary surgery was performed by my orthopedist colleagues but her thin skin couldn’t tolerate the pressure from the hematoma and started to die.

When I was consulted, her right knee skin was fully necrotic (dead).
I removed the necrotic tissue and covered her knee implant with the medial and lateral gastrocnemius muscle flaps (leg muscle) and skin graft from right thigh.

It was definitely a long journey for her but she healed so well that it is difficult to tell she had knee reconstruction.

Here is a video of SPY Portable Handheld Imager (SPY-PHI) showing blood flow in skin vessels of mastectomy flap (breast skin after breast tissue was removed). We first give patient intravenous indocyanine green and in couple seconds special camera allows us to visualize the blood vessels.
This mastectomy flap was very well perfused (the whole skin turned “white” pretty quickly) and I proceeded with reconstruction as planned.
