Recently opened in Johns Creek, Family Physical Therapy & Wellness strives to provide your family the highest quality of rehabilitation, therapy, assessments and modality services anywhere. We can treat people 0-100, whether you or your family member have been diagnosed with a syndrome, disease, condition, orthopedic injury, have pain, or any type of impairment, we will work closely with you and everyone involved with your care, to restore function and improve quality of life. Our mission is to take a holistic approach and look at the whole body while providing quality of care. At Family Physical Therapy & Wellness we guide you through the entire rehabilitative process, and teach you how to maintain the function restored and improved quality of life. Our goal is to create a warm, friendly, stress free environment where you feel welcomed and have the support you need to get through the process.

Dear valued patients,

Thank you for allowing us to provide you with physical therapy (PT) at this location for the past 3 years, and for referring your family, friends, and co-workers to us.
We have decided to not renew the lease as of April 30th, 2019, and will be permanently closing Family Physical Therapy and Wellness.

I will continue to provide physical therapy in Cu***ng, GA.

The new clinic address is:

They are in network with all major insurance plans, except Ambetter and Medicaid.

If this location is convenient to continue PT, please call the office and let them know you would like to continue your PT with me at the Cu***ng location. Since it is a different company, you will have to complete paperwork as you did for us, and they will verify your insurance as we did.
