Why does my upper back have pain when stretching?
Upper back pain during stretching is typically caused by acute muscular strain, underlying muscle tightness (especially in the traps or rhomboids), or irritation of the small facet joints in the thoracic spine. Why thoracic pain can move up to your neck?
▪️Shared muscles: Muscles like the upper traps, levator scapulae, and rhomboids run from your upper back into your neck and shoulders. If one area is tight or irritated, it can “pull” on the other.
▪️Posture chains: Slouching, forward head posture, or long desk/phone time loads both the upper back and the neck.
▪️Guarding/compensation: When your thoracic area hurts, you subconsciously tense your neck to protect it — that tension can turn into neck pain.
▪️Trigger points: Knots in the upper back often refer pain into the neck or base of the skull (and even cause headaches).
