Hidden Hollow Care Center is currently looking for a caring and compassionate leader to manage our Orem home for intellectually disabled individuals.
Responsibilities:
1. Supervises and coordinates Individual and Staff activities on assigned shift.
2. Assists individuals in daily living and personal hygiene training activities.
3. Coordinates and assists in serving individual's meals.
4. Implements mealtime procedures and trains staff.
5. Implements Behavior Reduction Programs according to policy
6. Lifts and moves non-ambulatory individuals.
7. Ensures the completion of daily observation sheets, log entries, daily reports, programmatic data, and schedules.
8. Maintains and monitors individual data in Therap.
9. Provides continuous training to staff on individual programs and policy and procedure.
10. Assists QIDP in developing effective staff/individual activity schedules.
11. Assists QIDP in writing self-care, daily living and basic academic assessments and skill acquisition programs.
12. Assists in changing linen, cleaning individual equipment, area and materials.
13. Completes and maintains inventories to ensure that individuals have adequate and appropriate clothing and toiletries, equipment and supplies, completing purchases requests as needed.
14. Requests maintenance as needed.
15. Promotes and maintains a warm, normalized homelike environment for the individuals.
16. Trains and ensures that staff is following proper policy and procedure within the home for safety, infection control and housekeeping.
17. Participates in staff interview process as needed.
18. Assists with completion of internal investigations as assigned.
19. Maintains adequate staffing requirements as designated by policy arranging for substitution as needed.
20. Designates staff for individual's appointments and field trips.
21. Attends and participates in standing committees as requested.
22. Documents all unusual incident reports and accident reports and notifies supervisor according to policy.
23. Arranges for delegation of critical duties when off duty or on approved leave.
24. Able to work with a diverse population of people.
25. Able to communicate clearly and professionally.
26. Able to write in a clear and professional manner.
27. Able to demonstrate an understanding of the data collection process.
28. Demonstrates and practices knowledge of facility evacuation and emergency procedures.
29. Provides leadership and supervision to assigned staff actively monitoring programs for compliance.
30. Reports instances of abuse in accordance with specific guidelines in the abuse policy.
31. Understands policy and procedures.
32. Attends scheduled trainings and in-services.
33. Able to demonstrate learned competencies to others.
34. Completes duties of Facility Supervisor as assigned.
35. Completes internal monitoring tools as assigned.
QUALIFICATIONS
*A minimum of one year experience in residential setting working with developmentally disabled.
*An Associate's Degree preferred. A High School Diploma or GED required.
*A minimum of one year experience in a supervisory capacity is required.
*Must possess a valid state driver’s license, with an acceptable driving record.
*Must be able to pass a criminal background screening.
*Must be capable of lifting 50 pounds and be capable of performing the physical characteristics of the job.
*Must have good oral and written communication skills.
