RHS is seeking a dedicated and compassionate full-time/part-time Community-Based Social Worker & Case Management Worker (CBRS/CM) to join our team. This position will serve clients in the greater Idaho Falls area, working with both children and adults. The CBRS/CM will be responsible for linking clients to services in the community, coordinating care, and providing vital support.

Key Responsibilities:

Case Management: Coordinate services and assist clients in accessing necessary resources, including helping arrange transportation.
Client Support: Collaborate with families, doctors, and therapists to ensure effective communication and care.

Skill Building: Provide instruction in various skills across different settings, including office, home, and community environments.
Documentation: Complete weekly client notes, 90-day reviews, and annual plans.
