Our plan includes:
• Timely virtual, follow-up with nurse practitioner after discharge
• Scheduling of primary care & specialist follow-up appointments
• Medication reconciliation & education while ensuring prior authorizations
• Personalized outreach to meet individual needs

Lara is a dedicated Nurse Practitioner with over a decade of experience in primary care and urgent medicine, specializing in chronic disease management and proactive health monitoring. Before joining AWP Healthcare, she managed patients with complex chronic conditions as a primary care provider and later led her own urgent care clinic, deepening her expertise in both routine and urgent patient care.

As a Board-Certified Nurse Practitioner, Lara is passionate about empowering patients through effective chronic care management, ensuring they receive personalized support to improve their health and well-being.
