Our Transitional Care Management (TCM) provides patients with a safe, supported recovery after hospital discharge. We coordinate care during this critical period to improve recovery, reduce complications, prevent readmissions, and enhance support for patients to regain stability at home.

Our plan includes:
• Timely virtual, follow-up with nurse practitioner after discharge
• Scheduling of primary care & specialist follow-up appointments
• Medication reconciliation & education while ensuring prior authorizations
• Personalized outreach to meet individual needs
