Seniors in Maryland can hire their own adult children to be their paid caregivers through the state’s Consumer-Directed Medicaid Personal Assistance Program.
This means a mother, father, aunt, uncle, or grandparent can choose someone they love and trust the same child they raised instead of a random agency worker. Maryland will then pay that child directly for providing daily care.
This is REAL, LEGAL, and one of the most under-used benefits in our state.

• The senior qualifies for Maryland Medicaid and needs help with activities like bathing, dressing, cooking, mobility, and more.
• They choose the Self-Directed option (NOT agency-directed).
• The senior hires their son or daughter to provide the care.
• A Fiscal Intermediary handles all payroll, taxes, and paperwork.
• The child receives direct deposit every 2 weeks as an official paid caregiver.
This program gives seniors dignity and independence while giving families financial support for the care they are already providing out of love.
💰 How Much the Child Can Earn
Maryland caregiver pay rates range from $16.00 – $18.50 per hour depending on the county.
👉 Example: 40 Hours per Week (Full-Time)
• $16.50/hr × 40 hrs = $660/week
• $2,857/month
• $34,284/year
👉 If the Senior Has Higher Needs (50–60 Hours Approved)
• $16.50/hr × 50 hrs = $825/week
• $16.50/hr × 60 hrs = $990/week
• Yearly range = $42,900 – $51,480
Seniors with dementia, wheelchair needs, or major mobility issues often qualify for higher hours.
📝 How to Apply in Maryland (Simple Steps)
1. Apply for or confirm Maryland Medicaid
Seniors 65+ can still qualify even with income Medicaid has special rules for long-term care.
2. Request an LTSS Maryland Assessment
Ask for evaluation for:
• CPAS (Community Personal Assistance Services), or
• Community First Choice (CFC)
Then say clearly:
“I want the SELF-DIRECTED option so my child can be my caregiver.”
3. Senior Chooses Their Child as Caregiver
