We were proud to share information about the Juniper Community Care Hub at the Grantmakers in Aging annual conference this week!

Yesterday, Trellis' Mark Cullen co-presented a session about the power of community care hubs to change lives and make care more affordable at the Grantmakers in Aging annual conference.
