We are grateful to St Christopher’s Hospice and Heather Richardson, former St Christopher's CEO and now consultant to the hospice and World Health Organization, for joining CHAP's Healthcare Leadership Workshop and for sharing her time, experience, and perspective with our community.

From our first meeting with Heather and St. Christopher's in London, it was obvious we needed to have her be a part of our Leadership Certification. Heather’s insights reinforced something we hold close at CHAP: strong leadership and meaningful collaboration elevate the quality of care, and when we learn from global leaders in hospice and palliative care, the entire field moves forward together.

Thank you for being part of this conversation and for advancing compassionate, person-centered care alongside us. We look forward to more opportunities to learn, connect, and grow together.

Accreditation should open doors, not create barriers. Yet too often, agencies face compliance burdens without support. CHAP is different.

For over six decades, we’ve been the only accreditor focused entirely on community-based care. We understand what agencies need—not just to meet standards, but to grow, improve, and lead.

Free access to CHAP’s standards: because quality shouldn’t be locked behind a paywall.

Direct access to CHAP’s Director of Accreditation and Quality Teams: so you get real answers, not just regulations.

Data-driven insights: helping agencies strengthen operations, improve care, and stay ahead of payer expectations.

Accreditation designed for real-world care delivery: flexible, innovation-forward, and built for the future of home-based healthcare.

Accreditation should be a roadmap to success: https://hubs.ly/Q03MD7rz0

According to a survey by The John A. Hartford Foundation, Age Wave, and The Harris Poll, 91% of older adults agree that their healthspan (the years they can live in good health) is more important than their overall lifespan.

Why does it matter?

This finding reminds us that aging well isn’t just about adding years; it’s about adding life to those years. Older adults want to stay active, independent, and engaged for as long as possible. That means healthcare should focus on treating illness and preserving function, mobility, and quality of life.

Age-Friendly Care at Home helps make that possible by aligning care with what matters most to each person; so they can live better, not just longer.

The home can contain hidden risks, and Clinical Managers prevent them from causing harm.

From incomplete medication lists to outdated care plans, small gaps in oversight can quickly lead to big consequences for patients, families, and agencies. The good news? These risks are preventable with strong leadership and the right systems in place.

In our latest blog, “The Hidden Risks in Every Home: Why Clinical Managers Are the Safety Net,” Dr. Jennifer Kennedy explores how Clinical Managers protect patients through sharper medication management, meaningful care planning, and stronger team accountability.

A single, accurate medication list can prevent harm, yet it’s often the messiest part of home visits.
We unpack how to turn a kitchen-table pile of bottles into a clear, living record that protects patients, reduces polypharmacy, and keeps agencies compliant. Drawing on decades at the bedside and in surveys, we walk through the moments where discrepancies hide—transitions between providers, “as needed” meds, herbals and supplements, dose tweaks after a clinic visit—and show how to bring everything into alignment with orders and what’s actually in the home.

Expect actionable checklists, questions to ask on every visit, and a reminder to have patients carry a current list to appointments and during any transition of care: https://hubs.ly/Q03QNHNW0
