If you're on a GLP-1 medication, there's something you should know that might change everything.

The dose you take is not the same thing as the level of medication in your body.

Every time you inject, the new dose stacks on top of what's still active from your last dose. And the last one before that. Your body doesn't reset to zero between shots. So the actual amount of drug working in your system is almost always higher than what your prescription says — sometimes dramatically higher.

This is why side effects tend to get worse over time on the standard protocol. It's not that you're getting sensitive to the medication. It's that the medication is accumulating faster than your body can use it up.

I know this because it happened to me.

My first time on a GLP-1, I followed the standard dosing escalation. At first, I felt great--but that didn't last. I got sick starting about week 3, and stayed sick a long time after that. Sure, I lost weight — but when I looked at the data, I'd lost more muscle than fat.

That wasn't OK.

So I did a deep dive and figured out what was actually happening in my body. I figured out how to calculate precisely how much medication was in my body at any given time, and then calculated exactly how much medication to take to make me feel my best.

It turns out, I actually needed a lot less the second time around. And the outcome was completely different. Same medicine. Freedom from dietary obsession and food noise. Absent side effects. Much more fat lost than muscle.

Once I figured out the ideal medication level for me—not the dose, but the level—and figured out how to personalize my dosing regimen to fit, everything changed for the better.

That experience became the foundation for what I've been building: MyLevel™ — a system that models the actual medication level in your body in real time and calculates the exact dose you need to stay in your personal therapeutic window.

At Voafit over the past few years, we've used this approach with 112 patients. When we benchmarked the results against the major clinical trials:

→ Our semaglutide patients lost weight ~1.3x faster than the trial — at about half the dose
→ Our tirzepatide patients lost weight ~1.1x faster — also at about half the dose
→ Not a single patient quit because of side effects

I get questions all the time about the best dosing interval for GLP-1s. Weekly, twice weekly, microdosing....etc.

Here's the best answer:

The amount of the dose typically determines how HIGH your medication levels get (and thereby your maximum appetite suppression and risk of side effects).

How often you dose typically determines how LOW your medication levels get before your next dose.

The closer your doses are together, the less variability there will between maximum medication levels.

At our clinic, we set a target medication level and our application allows us to calculate both the exact dose needed to hit that level with any individual dose, and also to set the preferred dosing interval depending on how tightly the patient wants their levels to be controlled.

I've attached two photos--both patients have the same target level (how high their medication levels get).

However, one patient doses once a week and has a lot more variability in their levels. This gives them more dietary freedom and flexibility towards the end of the week.

The other patient doses every 3 days. The peak level is the same--but they stay much closer to that line on average.

To summarize: need more appetite suppression? Increase your target level. Want tighter control? Take the dose needed to hit that level more often.

For the past few years, I’ve been working on a different way of using and prescribing GLP-1 medications.

In clinical practice, I kept seeing the same pattern. Some patients felt great and made steady progress, while others struggled with side effects, inconsistency, or eventually stopped treatment altogether. The difference often wasn’t motivation or discipline. It was how the medication was being used.

Most GLP-1 protocols rely on fixed dose schedules. But these medications don’t really behave in “steps.” They build and change in the body over time, and how someone feels is driven more by their medication level than the number on the prescription.

Over the past year, working closely with patients, we began using a level-based approach focused on identifying the medication level where each person feels and performs best. That work evolved into what is now called MyLevel™.

As this approach developed, we began noticing consistent patterns. Patients often experienced fewer side effects, more stable appetite control, and meaningful progress using much lower doses than are commonly used in standard titration schedules. Just as importantly, far fewer people felt the need to stop treatment altogether.

We’re now starting to share this approach more publicly with the goal of helping more people succeed with GLP-1 therapy safely and sustainably.
