What is Critical Limb Ischemia (CLI)?

Critical Limb Ischemia (CLI) is the advanced stage of peripheral artery disease (PAD) resulting in a severe obstruction of the arteries which markedly reduces blood flow to the extremities (hands, feet, and legs) and has progressed to the point of severe pain and even skin ulcers or sores. Foot complications in diabetes are common and account for more hospital admissions than any other diabetic complication. Foot ulcers are one of the most significant progressions of the disease and are associated with neuropathy (nerve damage) and/or peripheral vascular disease (poor circulation). These greatly increase the risk of amputation with up to 80% of amputations attributed to foot ulceration.

Venous disease is a progressive condition that may start out with mild to no signs. With time, it may lead to moderate or even severe symptoms such as heaviness, aching, swelling, tiredness, and itching, which can lead to a leg ulceration.

The CEAP Classification is a common system that is widely used to diagnose and identify the severity of varicose veins as they progress with time:

Clinical – Describes the physical appearance of veins
Etiologic – Whether the condition is inherited
Anatomic – Identifies the veins involved
Pathophysiologic – Evaluates the direction of blood flow

If self-care techniques such as exercise, lifting your legs while sitting and/or wearing compression stockings have not worked and/or you wish for cosmetic improvement of your veins, make an appointment to see your doctor to discuss next steps. It is especially important to seek help right away if you have developed a leg ulcer or other non-healing wounds.
