Weekly Imaging Case Quiz #21-1
Answer:
- Obvious radiolucent lesion in the tibial metaphysis
- Less obvious lesion in the lateral aspect of the medial femoral condyle

Firstly, we know this is a pediatric patient (open growth plates) and the tibial lesion involves the metaphysis of the proximal tibia. From the provided radiographs. It is mostly, if not entirely radiolucent and there is minimal, if any, bony expansion. There is no visible periosteal response or associated soft tissue mass. This does not appear to across the physis. These factors should help limit your differential diagnosis from a lengthy list of possibilities for a solitary radiolucent lesion in the metaphysis of a long bone prior to advanced imaging. The CT confirms this lesion has no internal calcified matrix and doesn't cross the physis. There is also an appreciation of a significant degree of endosteal thinning without visible breach of the cortex. The exact tissue diagnosis is not necessary, as the cellular makeup of this benign pediatric tumor will be identified after biopsy. Primary considerations include solitary bone cyst, osteoblastoma, eosinophilic granuloma, aneurysmal bone cyst, and fibrous xanthoma.

The CT also shows clearly the presence of the osteochondritis dessicans seen as a slight depression of the medial femoral condyle on the AP projection and as a subchondral radiolucency on the lateral radiograph.

Scheuermann (and yes, it is NOT Scheuermann’s) disease or juvenile kyphosis is classically defined by the presence of Schmorl nodes and decreased vertebral body height involving 3 or more thoracic levels associated with an increased thoracic sagittal curve.

Answer: Tibial stress fracture

In general, stress fractures occur when a repetitive load is place on the bone and there is a mismatch in the degree of force and the ability of the bone to sustain the degree of the load. There are two major categories:
1. fatigue fracture - increased stress placed on normal bone.
2. Insufficiency fracture - normal load placed on abnormal bone.

This case is a fatigue fracture in an adolescent runner. There is reactive sclerosis at the fracture with cortical thickening and solid periosteal new bone formation involving the posterior cortex of the proximal 1/3 of the otherwise normal tibia.
