What does it take to help older adults age safely and independently at home?

That question is explored in a recent episode of the Century Lives: The Home Stretch podcast titled “Capably Aging In Place.”

The episode features insights from:
• Sarah L. Szanton, Dean of the Johns Hopkins School of Nursing
• Allyson Evelyn-Gustave, CAPABLE Clinician Training Specialist
• Shauna Buchmoyer of United Disabilities Services in Lancaster, PA

Together they discuss how the innovative CAPABLE Program helps older adults remain safely in their homes through simple, practical changes—like installing grab bars, improving mobility, and addressing everyday barriers to independence.

With the nation’s population aging rapidly, programs like CAPABLE are helping communities rethink how we support older adults where they want to be most: at home.

For many patients, healing doesn’t happen in a hospital room—it happens at home.

A physician reviews a care plan. A nurse visits a patient. A team works together to help someone regain strength, independence, and confidence.

That collaboration is at the heart of the care provided through Colorado Visiting Nurse Association.

This National Doctors' Day, we thank the physicians who partner with our teams to ensure patients receive compassionate, high-quality care wherever they call home.

Your expertise and commitment help patients recover, remain independent, and live with dignity.

Every step matters. ❤️

At Colorado Visiting Nurse Association, our Community Medical Foot Clinics help older adults and community members stay healthy, mobile, and independent. Behind each clinic is a team of caring volunteers who make it all possible.

We are currently looking for dedicated volunteers to help support our clinics across the Denver Metro area. Volunteers assist with light clerical tasks and help ensure the clinics run smoothly — creating a welcoming experience for every person who walks through the door.

This is a meaningful way to give back while making a real difference in someone’s quality of life.

Volunteer commitment:
• Assist at 1–2 clinics per month
• Ability to travel to clinic locations around the Denver Metro area

We are currently seeking volunteers in:
Louisville, Lafayette, Boulder, Fort Lupton, Commerce City, and Golden.

If you’re looking for a way to share your time and compassion with your community, we’d love to hear from you.

The spirit of giving was on full display this holiday season thanks to the generosity of employees from Care Synergy, The Denver Hospice, and Colorado Visiting Nurse Association. Through employee-led efforts, teams organized a toy drive, a clothing drive, a blood drive, and delivered all the fixings for a holiday meal to families and patients in need. Giving back is at the heart of who we are, and this season made that clear.
