Reposted from 💥The first game-changing concept is that pain is not a signal that originates from bodily tissues. Rather, pain is now thought to be a highly motivating perceptual experience that seems to be generated on the basis of a perceived need to protect bodily tissue from harm.
💥The second game-changing concept is that pain is not a dependable indicator of tissue state. In contrast, somatosensory signalling is only one of the many, varied factors that influence the likelihood and features of pain. Furthermore, afferent signalling from peripheral tissues is subject to modulation at multiple points along its route to the brain, such that even the somatosensory signalling that arrives at the brain is a product of the nervous system’s plasticity.
💥The third game-changing concept arises from the second: the plasticity that is inherent to bodily systems supports a reversal of sensitisation processes and represents a viable target of treatment. This is particularly important for physiotherapy, precisely because physiotherapists are experts in using movement and learning to alter and train neural pathways, thus harnessing plasticity for clinical benefit.
☝🏻It is of paramount importance clinicians to understand these 3 concepts and learn about pain in order to provide better services and truly help others.

The ability to recruit high-threshold motor units increases more after strength training with a maximal effort on every rep compared to after strength training with a slow and controlled tempo (noting that such a slow and controlled tempo is not possible on the final reps of a set, which are necessarily performed with maximal efforts because of increasing fatigue).

A common flaw with the glute bridge exercise (and hip thrusts) is thinking that you need to go as far as possible, as far as your body will go.⠀

But keep in mind, the goal here is the glutes, not the low back. So the exercise should really be performed to extend you hips and NOT your back.⠀

To help with this, really tighten your core during the exercise and focus on squeezing your glutes. Then, simply stop the motion when the glutes are done squeezing. Resist the urge to continue by hyperextending at your back.⠀

🔎The post images are from a case report that documents the resolution of a lumbar disc herniation without surgery and demonstrates that pain can return when the apparent disc ‘injury’ is no longer present. Pain is complex!

🧠The MRIs shown here are from a 32 year old male who presented to the hospital with severe back pain in two different occasions. On October 2012, the patient did not have any neurological deficits, so physical therapy and a short course of oral steroids were ordered and his pain resolved.
-
👉In October 2013, another MRI was ordered when the same patient once again entered the hospital with severe back pain. This time around, his disc herniation was no longer visible.

⁉️What does this teach us⁉️
1️⃣Pain does not necessarily indicate tissue damage. In both situations documented above, the patient had low back pain. At the time of the first episode, a potential tissue insult could be identified. This was not true at the time of the second episode of back pain.
2️⃣Disc herniations can resolve with conservative care and time. The details of this individual’s rehab were not given, but it is important to know that relatively simple programs that include movements like walking, general low back mobility drills and low load resistance training exercises help the majority of people. A healthy dose of patience (up to 4-6 months) is important too.

Yesterday ⬅️ we asked what are your favorite ways to improve 🏃 ? This weeks study examined one of the many different interventions: backwards versus forward running.⁣⁣
The backwards running ✅outperformed✅ the forward group to a meaningful amount for #10-m sprint and counter movement jump.⁣⁣
This could be due to the fact that this may have been a novel stimulus for these subjects, as backwards running is not as common of a sprint training practice. It is interesting though that the backwards running performance ➡️ carried over to early stage forward sprinting. This could be due to the high nature of backwards running as this is typically associated with the early muscular 🦵demands from stage of sprinting. ⁣ ⁣
❗️Before we all start running backwards❗️the differences between these groups was not enough to give up traditional sprinting all together. However, this does show there can be some effectiveness to work it in. This is ✅especially✅ so given that most sports involve a TON of backwards and multi-directional running anyways.⁣⁣
Some 👎 limitations of this study include the age of these subjects (13-15) as pretty much throwing anything at them will make them better. The authors also ❌failed❌ to have any control for outside training such as other sports. We want to see these things addressed in future studies.⁣⁣

According to Ernlund & Vieira (2017), hamstring related injuries 💥 are the ❗️most common injury in sports❗️⁣⁣
What’s even more alarming 🚨 is a high reoccurrence rate which has been found to range from 12% to as ⬆️ as 33%! ⁣⁣
In a 2015 study by Opar et al, while ⬇️ hamstring eccentric strength was found to result in a 2.7x greater risk of future hammy injury, the authors also found a short fascicle length (especially in the long head of the biceps femoris) to result in a 4.1✖️ greater risk of future hammy injury! ⁣⁣
While studies such as Hickey et al (2017) have reported that eccentric loading / long length ex be delayed until achieving a pain free state, the authors of this week’s study note that guidelines, “lack firm scientific basis.” The authors further note that some studies such as Kilcoyne et al (2011) as well as Mendiguchia et al (2017) have used a ✅pain threshold✅ state during hammy rehab but have ❌not compared this approach directly to a pain free approach! ⁣⁣
