Hawkeye Care Center is a state-of-the-art care center located at 5575 Pennsylvania Ave. Give us a call for more information and to schedule a tour.

We offer a skilled and long-term care community with therapy, rehabilitation all on site. You can rehab and return home or stay with our family as you need. We understand that every individual is unique.

Trunk for Treat for families of residents and staff will be on Sunday October 26th from 1:30-2:30 p.m.

If you’d like to hand out candy, please bring 100-125 pieces of candy to hand out.
(Last year we had 97 kids and it grows every year!)
