For many years, the NEK Multi-County Health Department home health and hospice team has had the privilege of caring for patients and families across our communities. We are grateful for the trust placed in our staff and the care provided.
Beginning immediately, NEK is transitioning home health and hospice services so the department can focus its resources on core public health programs, including services such as WIC, immunizations, and other essential public health efforts. Home health and hospice care will continue through Amberwell Home Health and Hospice. NEK and Amberwell have a history of working together to improve community health, and this transition builds on an established collaborative relationship focused on patient care and local access.
Existing home health and hospice patients have already been contacted, and care is continuing with a focus on continuity and local access. Our priority throughout this transition is to ensure patients and families continue receiving the care they need.

How would you handle a household chemical emergency? 🚨 Store hazardous chemicals safely, keep them out of reach of children, and never mix or store them in food containers. Know the symptoms of toxic poisoning (difficulty breathing, eye irritation, changes in skin color, headache or blurred vision, dizziness, clumsiness or lack of coordination) and call the national poison control center at 800-222-1222 if someone is exposed. 📞
