The battle of thick skin. with rib graft in this young patient who has a severely crooked nose where the bottom of the septum was off by 1cm to the side. Not your cookie-cutter rhinoplasty nor anything to “preserve.”

Techniques included quadruple osteotomies (bone cuts using the ultrasonic ), spreader grafts, swinging door maneuver, triple septal extension grafts, clocking sutures, tip suturing, radix graft, medial and lateral alar base modifications, and a few more little ingredients of the secret sauce.

Can’t wait to see this patient heal. The nose is already swollen, which is temporary. What is permanent is the restored nasal breathing and structural integrity of the nose… and thereby the results. Swipe for more views 👉

One of the most popular procedures done in conjunction with a rhinoplasty is (drum roll please)… a chin implant!

When the chin is weak or recessed, the nose naturally looks larger. Strengthening the chin creates a more balanced profile and makes the nose look more proportionate without changing it any further.
I don’t recommend this for everyone, but I do perform chin augmentation in about 15–20% of my rhinoplasty patients.

Not everyone needs it, and not everyone wants it, but when it’s the right match, it can make a big difference in overall facial harmony.

African American rhinoplasty has its own approach as the nasal bridge usually needs to be raised rather than a hump removed. Plus, the thicker skin poses its own challenges.

This patient is 3 months out from her surgery and already looking amazing! With the nose only getting better as time passes. It will get narrower and more refined.

My focus here was to refine the bridge and tip while keeping the patient’s natural features and proportions intact.

Every rhinoplasty is unique, and it’s important to respect the nose's individual character while making subtle, balanced improvements.
