PPN vs TPN: quick vascular access reality check 👀

We hear this a lot:
👉 “It’s PPN, so a peripheral IV is fine.”

Sometimes.
But not always.

TPN is straightforward — high osmolarity = central access. No argument there.

PPN is where things get messy. Just because it can run peripherally doesn’t mean it should. If it’s running more than a few days, at higher rates, or on a patient who can’t keep an IV for more than 24–48 hours… that peripheral line is living on borrowed time.

This is why you’ll sometimes see a PICC placed even for PPN.
Not because we’re being aggressive — but because we’re being realistic.

Access decisions shouldn’t be based on labels.
They should be based on duration, osmolarity, vein quality, and patient safety.
