With nearly two decades of nursing experience, Kayla Hay brings compassion, skill, and servant leadership to her role as Administrator at Perkins Country Manor. A registered nurse for 18 years — including 13 years dedicated to hospice care — Kayla has built her career on providing comfort, dignity, and exceptional care to those she serves.

For the past six years, Kayla has expanded her impact as a nursing home administrator, blending clinical expertise with strong leadership to create a supportive environment for residents, families, and staff alike. Her unique ability to lead while staying connected to hands-on care reflects her personal philosophy: she will always be a nurse first.

Outside of work, Kayla is a proud mom of three children (ages 16, 13, and 7) and has been married to her high school sweetheart for 18 years — a relationship that reflects the same loyalty and dedication she brings to her professional life. Much of her free time is spent cheering from the sidelines at sporting events and making memories through travel with her family.

What Kayla loves most about Perkins Country Manor is its close-knit size, which allows her to lead with presence and purpose. The setting gives her the opportunity to not only guide the facility as an administrator but also remain actively connected to residents and staff in a way that reflects her nursing roots.

🌟 Amber Turner – Director of Rehab & Administrator in Training 🌟

Amber brings 11 years of experience as a Certified Occupational Therapy Assistant, serving in skilled nursing, outpatient, and home health settings. Her clinical background gives her a deep understanding of the rehab journey — and the real-life impact it has on our residents and families.
As our Director of Rehab and Administrator in Training, Amber is not only leading today — she’s helping shape the future of Perkins Country Manor. She has spearheaded the development and growth of our outpatient therapy program, expanding services and strengthening our connection to the community we serve. Her vision and drive have made a lasting impact.
Amber is passionate about taking care of our people and serving our community with excellence.
Originally from Ripley, Ohio, she is a proud mom of two who loves traveling, boating on the Ohio River, soaking up sunshine, and spending time by the water with her family.
Amber’s heart for care, growth, and community is evident in everything she does. We are incredibly proud to have her on our leadership team — and even more excited about the future she is helping build at Perkins Country Manor. 💙

🌟 Ruthie Bauer – Activities Director 🌟

Ruthie has been a Certified Nursing Assistant for 29 years, dedicating nearly three decades to caring for others. She joined the Perkins Country Manor family in December of 2023, and in 2024 she accepted the role of Activities Director — a position filled with love, joy, and connection, and if you know Ruthie, you know it fits her perfectly.
She brings warmth, excitement, and genuine compassion into everything she does. Whether she’s leading an activity or simply sitting beside a resident, her heart for people shines through.
Ruthie is a proud mother of sons and a loving “Mamaw” to many. She has been with her husband, Dennis Bauer, for 37 years — a beautiful testament to commitment and family. Outside of work, she enjoys riding horses and spending time with family and friends.
We are so thankful Ruthie chose to bring her decades of experience and her joyful spirit to Perkins Country Manor. Our residents are blessed by her every single day. 💙

At Perkins Country Manor, leadership isn’t just about job titles — it’s about commitment, consistency, and taking pride in the place you serve.

🌟 Jason Howard – Maintenance Director 🌟

Jason Howard currently serves as Maintenance Director at Perkins Country Manor. After working PRN in maintenance for four years, Jason returned full time two years ago and stepped into a leadership role, bringing a strong work ethic, hands-on expertise, and a deep commitment to keeping the facility safe, functional, and welcoming for residents and staff.
Jason has been married to his wife, Joy—who also works at Perkins Country Manor as the administrative assistant—for 27 years. Together, they share one son and value the close-knit family and community they’ve built both at home and at work.
Outside of work, Jason enjoys hunting, fishing, and spending quality time with family.
We are thankful for his leadership, his dedication, and the quiet strength he brings to our team. Perkins Country Manor is stronger because of him. 💙

🌟 Casey Brooks – Dietary Manager 🌟

Casey joined Perkins Country Manor in 2020 as a Nursing Assistant before eventually moving into the role of restorative aide. From the very beginning, her heart for our residents was obvious. She built relationships, showed up faithfully, and poured herself into the work.
Today, she serves as our Dietary Manager — a role that still keeps her connected to the residents she loves, while also allowing her to grow in leadership.
Casey says God knew she still needed her residents, so He opened the door for her to move up while also giving her more time with her five babies at home. And if you know Casey, you know that her family is her whole world.
Her journey is a perfect example of what it means to grow from within — staying rooted in compassion while stepping into new responsibility.
We are so proud of the leader she has become and so thankful she continues to build her future right here at Perkins Country Manor.

Some people step into a role. Others are called to it.

🌟 Sarah Applegate – Social Services & Marketing 🌟

Sarah has been part of the Perkins Country Manor family for three years. She began as our Activities Director, pouring joy and creativity into the lives of our residents. Today, she serves as Social Services Director and Marketing— continuing to be a voice, an advocate, and a bridge for the families we serve.
Sarah truly believes God led her to this job because He knew her heart needed it. And we believe that too. Her compassion, empathy, and ability to connect with residents and families is something you can’t teach — it comes straight from the heart.
She has been married to her husband Jeremy for almost 18 years, and together they have built a beautiful family with three daughters, one son, and one precious granddaughter.
Sarah brings her faith, her family values, and her heart for people into Perkins every single day. And we are so thankful she answered that calling.

🌟 Joy Howard – Administrative Assistant 🌟

For 10 years, Joy has faithfully served our residents and team, growing both personally and professionally alongside the very people we care for. She began her journey as a Certified Nursing Assistant, building her foundation on compassion, teamwork, and genuine connection.

In 2022, she transitioned into the role of Administrative Assistant, bringing her bedside experience into the office to better support our staff, residents, and daily operations. Because Joy has lived the work on the front lines, she understands our mission in a way that simply can’t be taught.

She believes in hard work, dedication, and building positive relationships — and you see those values in everything she does.

Outside of Perkins, Joy has been married for 27 years (to our Maintenance Director😂). Together, they’ve built a life centered around family and are proud parents who cherish their time together.

Joy’s loyalty, strength( literally, strength💪) and steady presence are part of what makes Perkins Country Manor feel like home. And we are deeply blessed that she chose to grow her career here with us.

🌟 Daisha Summers 🌟

Daisha has been part of the Perkins family for four years. She began her journey as a Certified Nursing Assistant and, through dedication and heart, has grown into serving as our Housekeeping Director, Asst Business Office Manager, and Outpatient Manager.
That kind of growth matters. It means she understands every layer of what we do — from hands-on resident care to keeping our environment organized, welcoming, and running smoothly.
Daisha takes pride in supporting our residents, patients, and staff while helping maintain the compassionate atmosphere that defines Perkins Country Manor.
Outside of work, she treasures time with her husband of eight years and their children — and that love for family is reflected in the way she serves ours every day.

🌟 Sheila Moon – Business Office Manager 🌟

For 23 years, Sheila has faithfully served as our Business Office Manager. In long-term care, that kind of longevity matters. She has walked alongside countless families, residents, and staff — offering steady guidance, compassion, and consistency.
Sheila has been married to her husband David for 22 years and is the proud mother and grandmother to a growing, beautiful family. When she’s not at Perkins, she enjoys traveling, cake decorating, crafts, and time with her grandkids.
Sheila often says she feels blessed to work with such incredible co-workers — and we feel just as blessed to have her. At Perkins Country Manor, we don’t just care for residents. We build relationships that last.
And Sheila is a true part of that legacy. 💙
