Our Giving Tree Is Back!
Each year, we are touched by the generosity of those who ask how they can brighten the holidays for our residents. We’re excited to announce that our 2025 Giving Tree is now up and ready!

This year’s theme is The Grinch, adding a playful twist that brings smiles to residents and visitors alike.

On the tree, you’ll find tags with our residents’ names and their Christmas wishes. If you’d like to spread some holiday cheer, simply choose a tag and sponsor a gift. Your kindness helps make the season truly special for those who call our community home.

These smiles from our Halloween celebration were worth the wait!
What a spooktacular time we had at our Halloween party! Residents and staff dressed up for the occasion, enjoyed treats and laughter, and even had a surprise visit from “Elvis” himself for some rockin’ musical entertainment!
Thank you to everyone who joined in the fun and helped make this Halloween one to remember!

Today, we pause to honor the brave men and women who have served our country with courage, dedication, and sacrifice.
We are deeply grateful to the veterans within our Augusta Health and Rehab family — residents, staff, and loved ones alike — who have given so much in service to our nation.
SPC Boone, Personal Care Worker at Silverleaf Assisted Living, presented each veteran with a Certificate of Honor, and Ashley from St. Croix Hospice joined us to pin each veteran in a touching tribute of appreciation.
We are deeply grateful for the service and sacrifice of our veterans — today and every day. ❤️🤍💙

Our Story

Augusta Health and Rehabilitation is a senior living community well-known for skilled nursing care, assisted living, and rehabilitation services. Within a continuum of care, our thoughtful team supports those needing assistance with daily activities, medical needs, and end of life care. Residents have the security of knowing they can remain here without having to relocate and separate from their familiar surroundings and friendships, should their needs change over time.

We are committed to, and legendary for, a high level of care with a personal approach. We accommodate the needs of every individual, making their experience as comfortable and memorable as possible.

Residents at Augusta Health and Rehabilitation have three delicious meals prepared for them each day. We also have a chapel on site for residents to utilize, as well as scheduled services for residents to attend to promote spiritual health. Additionally, Augusta Health and Rehabilitation has beautiful courtyards and outdoor spaces for our residents to enjoy.

We will happily provide the opportunity for you and your family to get to know our dedicated staff. Together, we’ll work on transitioning each new guest into one of our comfortable, new, state-of-the-art facilities.
