Duties and Responsibilities
• Communicate daily with interdisciplinary team including physicians, social worker, chaplain, nursing assistants, volunteer coordinator, and other hospice nurses
• Frequently visit patients and evaluating their condition
• Give support in accordance with individual patient’s needs
• Collaborate with caregivers in order to provide the best possible patient care
• Assist in administering medication
• Alert physicians about changes in patients' condition
• Help patients and their families with administration and paperwork
• Communicate all pertinent information and evaluations through clear and concise documentation
• Educate patients' relatives on coping techniques and provide comfort and support

Requirements
• Previous Hospice experience a plus
• In-depth knowledge of nursing care operations and administering medication
• Familiarity with safety regulations
• Keen observation skills
• High ethical standards
• Compassionate nature and excellent communication
• GA or multi state RN License
• Current CPR certificate

Hands of Grace Hospice is looking for skilled CNAs (Certified Nursing Assistant) to become a part of our facility’s compassionate care-giving team. The CNA will interact directly with patients, monitoring their vital signs and assisting with daily living needs. We are searching for a dedicated, compassionate and energetic individual to be a vital link between our patients and the nurses and other facility staff.

Responsibilities and duties
• Provide patients with bedpans, help with baths, shampoos and showers
• Provide adjunct care for the patient, including massage, companionship, therapeutic baths
• Check vital signs and record daily information in the patient’s chart
• Assist the nurses and other staff as needed
• Adhere to professional standards, follow policies and procedures and abide by federal, state and local requirements, as well as Joint Commission standards

Qualifications and skills
• High School diploma or GED
• Completion of a state-approved CNA certification training course
• Basic computer skills
• Ability to work as part of a team and to handle multiple tasks safely and effectively
• Good communication skills, including the ability to communicate sensitive information with empathy (“bedside manner”)
• High tolerance for working under pressure and handling critical situations with a cool head
