Introducing Foundational Succession Planning for Families!

Foundational Succession Planning involves making a plan for the future of a family member with a disability. It often includes the process of passing over family leadership roles. For some families this may mean planning for another family member to take over caregiving responsibilities. For others, this may mean having to rely on more formal types of services and support options.

Generously funded by the Colorado Developmental Disabilities Council (CDDC), the Colorado Office of Employment First (COEF) is offering 50 parents or guardians, age 65 and up, individualized foundational succession planning.

This planning will take a person-centered approach that will focus on defining and connecting individuals, caregivers, and families with the essential and next steps when considering future care arrangements for their family member with an Intellectual and Developmental Disability (I/DD), once the caregivers and/or family are no longer able to be primary supports for the person.

The end result will start a plan for the future of a person with a disability.

We are a family owned business with Tom, Chrissy, and Crystal. We serve all areas surrounding Denver-Aurora, Broomfield, Lafayette, Thornton, Westminster, Lakewood, Idaho Springs, Castle Rock, Parker, Elizabeth, Boulder, Louisville, Plateville, Erie, Longmont, and even Greeley. We provide home care services for disabled adults with physical or psychological disabilities.

We have close to 100 clients and 80 staff and family caregivers as of March 2019. We try our best to provide you with the services you or your loved one needs. We hire caring personnel that understand and have the empathy of those we provide services for. We perform Colorado Bureau of Investigation, DORA, and Colorado Adult Protective Services background checks on all personnel hired.

Our family has much experience in the healthcare field for over 25 years with both Chrissy and Crystal having nursing and home health experience. Chrissy was also a Case Manager for Medicaid and Medicare clients for the state of Florida and Colorado. Both have many years of experience in caring for those with disabilities from young to elderly.

We take Medicaid and private pay. Call us for any questions.
