We offer a full suite of services including Residential Services, Day and Seniors’ Choice activities, Family Caregiver, Supported Employment, Home and Community Support, and Supported Employment. Continuum’s Day Activities services cater to varying interests and abilities. Our Residential department similarly offers a variety of placements with supervision and independence levels tailored to the strengths and needs of the person served. In our Supported Employment program, our Employment Consultants work one-on-one with the individual to promote independence in all aspects of their job. Home and Community Support services include Personal Care, Respite, Supported Community Connections, Community Connector, Homemaker, Mentorship, and Massage Therapy and are provided by professionals who complete extensive training specific to individual needs.

Our Residential department similarly offers a variety of living options with supervision and independence levels tailored to the strengths and needs of the person served. In our Supported Employment program, our Employment Consultants work one-on-one with the individual to promote independence in all aspects of their job. Home and Community Support services include Personal Care, Respite, Supported Community Connections, Community Connector, Homemaker, Mentorship, Massage and Movement Therapy and are provided by professionals who complete extensive training specific to individual needs.
