VNA Health Care (VNA) is a humanitarian, not-for-profit organization providing medical services. This institution is an equal opportunity provider.

VNA Health Care is a Joint Commission accredited Federally Qualified Health Center (FQHC) that is certified as a Primary Care Medical Home. VNA Health Care has fifteen (15) health centers serving patients in Kane, Cook, DuPage, and Will Counties. Services include: comprehensive primary healthcare; wellness and annual preventative services for men, women, and children; sick care, treatment of chronic diseases and acute illness; free pregnancy testing & complete pregnancy care, including obstetrical services; pediatric care; mental and behavioral health care; podiatric care; massage and physical therapy; pharmacy and 340-B Pharmacy Assistance Program; laboratory testing; dental services; and case management and care coordination. VNA also provides community wellness services including: WIC (A Special Supplemental Nutrition Program for Women, Infants & Children), Healthy Families America home visitation program & Doula services, and Better Birth Outcomes. During this cold weather, remember that VNA offers TELEHEALTH appointments if your condition does not require an in-person assessment. You can request a telehealth appointment by calling (630) 892-4355 or request an appointment through your MyChart account.
