I sit here closing out my day and come across this thoughtful letter sent from a grateful patient. I love my job.

“Doctor Jimenez and the Ortho Staff:

I wanted to tell you what a wonderful, awesome benefit having this new hip joint is to me! Though you do these week after week, and I’m sure it is tiring and hectic, I wanted to take a moment to remind you of the great transformation the work you are routinely performing has done in my life. Never forget the enormous power this service — this technology you are working with — has to change the next 10, 20, 30 years of a persons’ every step! What was once a terrible, grimacing pain every hour is now gone. Thanks to you, Doc, and your Staff.

Sincerely, a happy hip replacement patient (you know theres this thing called HIPPAA)”

I was recently invited to teach a national corporate team from Depuy Synthes. I had a great time helping to train the Depuy Synthes marketing team and giving insights into how a surgeon thinks during a anterior hip replacement. I also enjoy having surgeons visit with me during some of my procedure days to understand some of the intricacies of the anterior hip replacement.

As a joint replacement surgeon it is important to recognize when an implant is loose and causing pain. This patient was complaining of pain in a knee that had been replaced several yeats ago.

I performed a revision joint replacement where the implants were completely debonded from the cement that is used to fix the metal implant to the bone (see video).

In the revision surgery I used implants that had longer stems that are cemented further into the bones for better fixation and stability. I also placed a porous metal cone in the tibia that allows for the bone to bond with a portion of the implant that can improve the long term fixation of this implant.

This patient is over a year out from surgery and was now finally happy with her knee.

Taking call at hospitals can lead to some challenging cases as demonstrated here. This patient came in with a hip fracture and a history of previous trauma to her femur (thigh bone)

At first glance it appears to be a straight-forward case, but the lower leg had a varus (bow-legged) deformity and pro-curvatum (bent position) of her lower femur. This complicates decisions as the standard implant for this is a long nail entered into the femur bone through the upper hip region. This usually extends to the lower femur.

I extended the implant as low as possible ending at the deformity. Here you see the end result after a couple of years healing.
