As this year comes to a close, meet some of my lovely patients from 2025! (And swipe to see 👉🏼 their text messages!)

What I see in my practice isn’t vanity. People want to feel like themselves again. More aligned with the version of themselves they recognize on the inside.

In psychology, there’s a concept called the Self-Discrepancy Theory: the gap between how we see ourselves today and how we wish to or used to look. When that gap widens, it can quietly affect one’s inner acceptance and confidence.

These texts from my patients mean everything to me - because their surgery has produced more than an improved appearance - it has created real internal happiness. THAT’s the outcome that really matters!

Grateful for my patient’s trust, and for the science, artistry, and psychology that intersect in this work every day. ✨🧠👁️‍🗨️

Fat repositioning **with recycling** enhances blepharoplasty ✨

This patient had an upper and lower blepharoplasty + a brow lift, using one of my favorite techniques: fat repositioning & fat recycling ♻️

✨ Lower eyelid fat was repositioned downwards to fill the orbital retaining ligament hollow and enhance cheek volume
✨ Upper eyelid fat was debulked, minced, and re-injected into the cheeks for improved symmetry and contour

I recently presented my technique at the Barcelona Oculoplastics meeting… I’m really excited about the power of this procedure!

Ever put your hair into a tight ponytail and liked the lift it gives your eyes and face? That’s the concept behind the !

I was asked to explain this procedure in a recent article. The “Ponytail Facelift” is designed for younger patients looking for a natural-looking rejuvenation with mostly hidden incisions.

How is this different from a “traditional” facelift?

So excited to share this amazing result! Only two months post-op, and this lovely patient is already glowing. Great surgery should just make you look like the best version of yourself. So grateful she agreed to share her results - lasering the eyelids is so powerful and natural!

Today, Nancy Carrejo RN retired from our operating room after 15 years.

I first met her when she was my wife’s postop recovery nurse in 2010. I was so impressed by her care that we immediately hired her for our own TOC operating room, where she has been a super caring and incredibly effective nurse for the last 15 years, touching patients lives positively every single day.

She’s a warm and loving person, and we are going to miss her terribly! But she has an exciting retirement awaiting her, and we are so happy for her! 💗

Check out this incredible transformation! This beautiful lady is just six weeks post-op, and she already looks stunning. What’s the most commonly request right before patients go under anesthesia? “Please make sure I look natural!” And of course it’s my core focus - to produce that sweet spot of real change that looks totally natural.

She underwent:

As Oculofacial Plastic Surgeons, we perform many non-cosmetic procedures to restore normal function of the eyes - this is a case of Thyroid Eye Disease – an autoimmune condition that causes inflammation in the orbit (eye socket) leading to bulging eyes, discomfort, and even vision loss.

She underwent bony orbital decompression surgery, which expands the orbit and allows the eye to sink back into a more normal position - improving both comfort and appearance. These surgeries can be life-restoring, and is one of the most rewarding reconstructive procedures we perform for our patients!

LED light masks are having a moment BUT do they really work?

The marketing is compelling. My honest opinion is that they are not “miraculous” devices, but the science is real and long-term use can deliver benefits to those who appreciate details.

The takeaway: At-home LED masks can be effective - but only if they use a clinically relevant wavelength and have sufficient irradiance (power).

Here’s a quick breakdown of benefits:
🔴 Red Light: Boosts collagen, reduces inflammation, and calms redness.
🔵 Blue Light: Treats acne and helps with hyperpigmentation.
🟢 Green Light: Soothes facial redness and rosacea.

Here’s a question I get asked all the time: “Why do I get dark circles under my eyes?” The answer is that it’s not about being tired…

On Peter Attia’s podcast (The Drive #355), I explained the 4 main causes of infraorbital (under-eye) dark circles:

1️⃣ Visibility of the orbicularis muscle
2️⃣ Shadowing
3️⃣ Hemosiderin (iron) deposition
4️⃣ Hyperpigmentation

Years ago, I published a review article on this topic that has been cited over 100 times (you can download it for free on Google Scholar/PubMed and in my Linktree). The bottom line is that each of us has a unique combination of these four causes, and analyzing each human’s specific factors is the first step toward the correct customized treatment.
