FILLER DISSOLVING: WHY PRECISION MATTERS

Filler dissolving is often perceived as simple — but in many cases, it is more complex than expected.

Although hyaluronic acid fillers are biocompatible, the body still recognizes them as a foreign substance. As a result, fibroblasts create a fibrous (connective tissue) capsule around the filler 🧬

👉 This creates a barrier.

Why dissolving may not work properly:
• Product may not reach the filler
• It can disperse into surrounding tissue
• Filler may remain even after multiple sessions

🔬 Why ultrasound matters

Ultrasound allows:
✔️ Precise visualization of the filler
✔️ Identification of fibrotic capsules
✔️ Accurate needle placement
✔️ More effective dissolving

Without precision, results can be incomplete — which is why many patients still feel filler after prior dissolving.

💉 Not all fillers dissolve the same

• Denser fillers (e.g., Voluma) → more resistant, may require multiple sessions
• Softer fillers (e.g., Volbella) → typically easier to dissolve

Correct approach always depends on the product used.

👄 Why lips are different

Lip tissue behaves differently:
• Better microcirculation
• Less fibrotic capsule formation

That’s why lip filler is often easier to dissolve without ultrasound.

However, if filler is present above the natural lip border, fibrous tissue can still form — and ultrasound may be needed.

Why Does the Temporal Area Change With Age — And How Do We Treat It?

As we age, the face goes through a process called skeletonization. This means:

• Bone remodeling occurs
• Fat pads diminish
• Ligaments weaken
• Skin becomes more lax
• Muscle structure changes

These changes make certain areas of the face appear more prominent — including the zygomatic bones, jawline, but some areas like temporal region develops concavities. This is why some patients feel they look more tired or notice hollowing in the temporal area.
