The perfect fit is loose enough to get a few fingers under it easily, but tight enough it can’t reach the mouth. If you are unsure, the best option is to measure. By the time kids reach 4 they can usually remove it on their own and length becomes more about comfort than safety. 🙂
In general, estimated lengths would be
11.5” for 2-4y
12” for 3-5y
12.5 for 4-6y
13” for 4-7”

All measurements within 1/4 an inch.
