👩‍⚕️ What You'll Do:
Home visits, evaluations & assessments
Create care plans & coordinate with physicians
Teach patients and caregivers
Keep accurate clinical notes/ Maintain accurate & timely documentation
Attend case conferences & trainings/meetings
📌 Requirements:
Active LVN license (California)
1+ year of LVN experience- Minimum
Valid driver’s license & auto insurance
Current CPR, First Aid, & IV Certification
Able to pass background check, drug screen, physical
🗓 Schedule:
Monday to Friday
Weekends + On-Call rotation
👨‍👩‍👧‍👦 Patient Types: Adults, Geriatrics, Families

This is an outstanding accomplishment and a reflection of Jina’s deep clinical expertise, strong leadership, and unwavering commitment to delivering exceptional patient care.
