The Piof Home Health Care : Supportive Care Coordinator is responsible for the day to day management of the schedule for the medical staff within the geographic region they are assigned
Their primary focus is to process referrals and schedule admissions to ensure that the medical staffs have what they need to conduct their visit
The Coordinator provides timely and appropriate medical staff staffing for all patients visits
The Coordinator must be friendly, courteous, and helpful to patients, families, and referral sources
They must provide appropriate information when assigned and complete all assigned work

Minimum two years' experience in hospice and/or home health care personnel scheduling demonstrated ability to multi-task efficiently and effectively in a busy, stressful environment with attention to detail and accuracy.
