🎉 Celebrating 2 Years of House Calls! 🎉

Tomorrow marks two years of providing geriatric primary care through home visits, and I’m incredibly grateful to the patients and families who have trusted us with their care. It started with an idea, one patient and a need in the community. It’s brought me immense joy to develop this practice.

As we look ahead to the new year, I’m excited to share that we’re growing our practice and hiring additional clinicians to help meet the complex medical needs of older adults—right where they’re most comfortable: at home.

• Chronic disease management
• Medication review
• Post-hospital follow-up
• Care coordination for complex health needs
