Early hospice care offers more comfort, connection, and dignity for the patient; more time to fulfill meaningful wishes; and more support for caregivers and families throughout the journey.

As part of Giving Tuesday on December 2, the SSM Health St. Clare Foundation - Baraboo is raising funds to support the SSM Health Hospice Home at 915 12th Street, as well as residents with financial needs.

The facility is open to hospice-eligible patients whose symptoms can be managed in a residential setting but are unable to have those needs met at home.

Our team of caring individuals is trained to meet the physical, spiritual, social, and emotional needs of patients, and every effort is made to provide comfort and enhance the quality of life for patients and their families. While staying at one of our hospice residential facilities, residents receive all the in-home hospice services plus meals, medication administration, laundry, and housekeeping. The facility is staffed 24 hours a day.

In addition, thanks to the Topham Family Foundation, every gift will be matched dollar-for-dollar up to $5,000.
