Resolutions Hospice Bastrop is actively seeking a RN Case Manager to support our patients in Bastrop and the surrounding areas including Fayette, Caldwell and Lee counties. We have a dynamic team and a rapidly growing census. We are looking for a Case Manager who loves hospice, is passionate about patient care, able to work in a collaborative environment and ready to help us grow. This is a fulltime, salaried position with competitive compensation, a full benefits package and employee enrichment including tuition reimbursement, referral programs and more.

Case Manager Responsibilities (include but are not limited to):

Complete nursing assessment and update plan of care, including the patient and the family in the planning process.
Provide direct Hospice nursing care to patients in order to maintain highest level of comfort and quality of life, and regularly assesses for patient nursing care need
Educate families and caregivers of the patient on recognizing potential symptoms and providing safe and competent care for the patient.
Offer emotional and practical support for both the patient and their family or caregivers
Create strong partnerships with community organizations in patient care.
